New IBM xSeries firmware and device driver file naming convention

This device driver version supports DS4100, DS4200, DS4300, DS4400, DS4500, DS4700 and DS4800 storage subsystems only. Please review the attached readme file for the drive usage requirements and restrictions 3.03.19 (FC-2 and FC2-133 HBAs), 4.00.18 (FC 4Gbps FC HBAs)

• Provide support for the following DS4000 storage subsystems - DS4100, DS4200, DS4300, DS4400 and DS4500.
  
• Fix the Logical Drive (LUN) not available in the Host Direct-IO attached configuration,where the server FC HBA ports are directly connected to
  the DS4000 storage subsystem ports, as documneted in the Fibre Channel Storport device driver version 9.1.2.16 readme file.
  

Version 9.1.2.16 05/24/2006 (QL - 03/16/06)

This device driver version supports DS4700 and DS4800 storage subsystems only.

• RISC code 4.00.18 (FC 4Gbps FC HBAs)
  
• Provide support for the new IBM DS4000 4Gbps single and dual port Fibre Channel
  
• Set additional FW option bit 10 for IBM FC-2 and FC2-133 HBAs so that FW waits for the task management command e.g. Reset Lun to complete.
  
• Fix OPT226506: Hba does not relogin to target after target sends LOGO to Hba
  
• Fix Er44735: The link status behave differently between QLE2460 and QLA2342
  
• Fix Er45228: Incorrect HwVersion string is reported by QLogic IHV
  HBA_GetAdapterAttributes call
  
• Fix Er45230: Event Id 11 entries logged by ql2300 at offset x10 and offset x34 do not match the Error Codes published on the web
  
• Fix Er45232: Use memory map mechanism in the driver

• Fix 90818 (9.1.1.10 QL 3.03.18 4.00.12): FC HBA device driver returns selection Timeout immediately after cable pull in a DS4000 storage
  subsystem with dual FC path configuration.
  
• Fix 90652 (9.1.1.10 QL 3.03.18 4.00.12): FC HBA device driver returns a selection timeout when rebooting a controller iin a DS4000 storage
  subsystem with dual FC path configuration.
  
• Embed IBM DS4000 4Gbps single and dual port Fibre Channel HBA FW in the device driver
  
• Fix ER43175 (9.1.0.11 QL 3.03.16 4.00.12): New driver could not detect any device to install as BOOT from SAN in certain configuration setup. Investigation showed that driver detects the target device successfully but failed the logins because the target had previously been logged in
  through by the HBA firmware. Driver is changed to handle this particular case correctly.
  
• Fix ER42601( 9.1.0.10 QL 3.03.16): Need to change bit used to control LED mode.
